Tandigm Health, the Philadelphia-area primary care venture started last year by Independence Blue Cross and Davita HealthCare Partners, has inked a deal with the Holy Redeemer Health System. Effective March 1, 75 primary care providers within Holy Redeemer's Innovative Wellness Alliance will join Tandigm, which aims to incorporate real-time analytics into a care model similar to the patient-centered medical home.
