Analyzes Major Efforts to Predict and Reduce Hospital Readmissions, And Presents A Novel Framework for Converting Pitfalls Into Opportunities Using Software
RALEIGH, N.C.--(BUSINESS WIRE)-- Axial Exchange, Inc. announced that its CEO Joanne Rohde will present new insights on how technology can be used to optimize initiatives to reduce hospital readmissions – and the implications of this for CMIOs and health care executives as part of this week’s Healthcare Information and Management Systems Society (HIMSS) Texas 2012 conference in Galveston TX.
Rohde will present to health care executives and practitioners, many of whom are struggling to meet the rising bar for health quality. Presenting new research and insights alongside Rohde will be representatives from the University of Texas Health Science Center at Houston and healthcare analytics firm Clinfometrics of Boston. Rohde will demonstrate how technology can be used to accelerate the results from implementations of Project RED from Boston University and Project BOOST from the Society of Hospital Medicine.
Citing the work of major Southeast medical institutions such as WakeMed and others, Rohde will highlight eight key ways that health IT teams can put data they already own to use in enabling a better discharge process – from expediting transmissions of discharge charts, educating patients, confirming medications and organizing post-discharge services, among other steps.
