Aledade adds 500 primary care practices to its network for 2025

Aledade has added more than 500 primary care practices to its network for 2025, the value-based care company announced.

With the new additions, Aledade now works with more than 2,400 PCPs across the country, which serve close to 3 million patients in value-based care arrangements. This includes participants in the Medicare Shared Savings Program, Medicare Advantage, Medicaid and commercial plans.

Aledade said that 83% of its new MSSP participants are joining the program for the first time. Nearly one-fifth of new entrants to MSSP are supported by Aledade.

“Since day one, Aledade has led the charge in enabling independent primary care doctors nationwide to adopt and thrive in value-based care, enhancing patient outcomes while ensuring practice sustainability,” said Farzad Mostashari, M.D., co-founder and CEO of Aledade. 

“With every new practice that joins the Aledade network, we gain a greater ability to improve patient outcomes, support practice growth and move health care forward nationwide," he said.

In addition to MSSP, Aledade has increased its participation in the ACO REACH program by 13% and is a key player in the new Primary Care Flex program, according to the announcement.

Aledade's network reaches into 46 states and the District of Columbia, the company said, and includes primary care organizations of many different sizes. More than 300 community health centers, most of which are located in areas with health professional shortages, are part of that tally.

For 2023, Aledade's ACOs generated $801 million in savings, the highest of any ACO partner in the U.S. Its partner organizations have driven $2 billion in savings since 2014, the company said.
