Humana taps Monogram Health to offer value-based kidney care in 4 states

Humana is teaming with Monogram Health to offer value-based kidney care in four states.

The insurer said this week that the value-based care arrangement would include most of its Medicare Advantage (MA) HMO and PPO members in Tennessee, Alabama, Louisiana and Mississippi. Through the partnership, eligible members can access Monogram's specialized offerings, including in-home primary and specialty care, medication management, dialysis and transplant coordination, palliative care and social services.

Patients with end-stage renal disease are able to enroll in MA as of last year, and plans are designing programs that can better manage often-complex kidney needs. MA plans also have greater flexibility than traditional Medicare to target members' social health needs.

“With millions of Americans living with chronic kidney disease and the number continuing to grow, Humana is committed to increasing access to evidence-based, multi-specialty kidney care to ensure our members have the services they need without having to leave home,” said Carl Daley, Humana's senior vice president of retail strategy and operations, in the release. “Building on our existing relationship with Monogram is an important way we can provide the highest levels of support and access to care for our members.”

Humana has also invested heavily in value-based care relationships, and, as of June, more than 3.2 million of its MA members are cared for in value-based models. This accounts for 75,600 primary care providers in more than 1,000 value-based arrangements across 46 states and Puerto Rico, Humana said.

In addition, the partnership expands on the existing relationship between Humana and Monogram Health, executives said.
