Open enrollment for the Affordable Care Act's marketplaces opens on Nov. 1, and with more turbulence on deck, insurers are offering a look at their plans for the coming year.

An August analysis from KFF found a median premium increase of 15% in 2027. That comes as insurers navigate the ongoing impacts of the expiration of the enhanced premium tax credits alongside significant changes implemented through multiple federal program integrity rules.

Those rules have faced legal challenges, but plans have still needed to account for the changes included therein. KFF said that the 15% increase makes for the second year in a row with a double-digit median increase. For 2025, the median proposed increase was 18%.

With that backdrop, here's a look at what some of the leaders in the exchange market will offer in 2027.

Centene Corporation

Centene's Ambetter Health unit, the brand for its marketplace plans, will expand into 55 new counties for the 2027 plan year, the company announced. Ambetter currently covers about 3.5 million people across 27 states and 1,700 counties.

The company will add a mix of new, condition-specific plans under the Ambetter Health Premier umbrella, including plans in North Carolina and Ohio that target patients with congestive heart failure. Members that enroll in these plans will see $0 copayments for certain heart failure medications as well as up to four $0 cardiologist visits per year.

The plans also include $0 copayments for certain mental health medications and nicotine replacement therapy, as well as coverage for key cholesterol labs, EKGs, cardiac education programs and nutritional counseling.

In addition to the new plans, Ambetter is expanding the reach of existing plans for individuals with diabetes and asthma or chronic obstructive pulmonary disease. The diabetes care plans are currently available in Florida, Tennessee, Texas, Alabama and Oklahoma, and Centene will add Louisiana, Mississippi and Indiana for 2027.

In these plans, members can access $0 preferred insulin as well as $0 copayments for certain medications for managing diabetes, high blood pressure, high cholesterol and mental health conditions. The plans also offer no costs for certain diabetes supplies and routine A1C lab services.

The plans for asthma and COPD, meanwhile, include $0 copays for preferred asthma or COPD medications, pulmonology visits, allergy tests and pulmonary rehabilitation services.

In Ambetter's traditional plans, members can access virtual care around the clock, and in participating states, the My Health Pays rewards program that offers annual rewards for completing certain health challenges.

"We are committed to delivering coverage options that reflect the diverse healthcare needs of the communities we serve," said Katherine Kirby, president of Ambetter Health, in the announcement. "Our condition-specific plans have delivered strong member satisfaction and primary care engagement, demonstrating the value of more personalized approaches to coverage."

Oscar Health

A new offering for the coming year is a plan called RetireEase, which aims to support the transition from early retirement to Medicare enrollment. The plan will be available in five states: Arizona, Florida, Ohio, South Carolina and Texas.

Under this plan, members can continue to see their trusted providers, and will see set prices for key services like preventive care, labs, physical therapy, heart care and bone-and-joint care. The plan includes no cost generic medications and supports for caregiving, transportation and the transition to Medicare.

The company is also expanding existing plans that target particular life stages or conditions. For example, the HelloMeno plan, which is designed for women navigating menopause, will grow to 15 states.
