XLHealth's Chronic Condition Special Needs Plan Becomes the Largest in the U.S.

100,000 Medicare Beneficiaries Discover Benefits of Special Needs Plans

BALTIMORE, Feb. 1, 2011 /PRNewswire/ -- Today, XLHealth, a leading provider of health plan solutions for Medicare beneficiaries with special needs, announced the enrollment of its 100,000th member, making Care Improvement Plus – the company's chronic condition Special Needs Plan – the largest in the country.

Care Improvement Plus' chronic condition special needs plan combines medical coverage – hospital and doctors' visits – with tailored Medicare Part D prescription drug coverage and care management support for Medicare beneficiaries with diabetes and heart failure.

Care Improvement Plus members and their caregivers are supported under the plan's model of care, which coordinates care across numerous providers and settings. The plan's growth demonstrates that chronically ill seniors are increasingly turning to health plans that meet their complex needs and emphasize care coordination, prevention and wellness to improve their quality of life.

"I feel fortunate to have discovered Care Improvement Plus," says member, Mary Carter of Columbia, Missouri. "I appreciate the support their nurses and other staff provide to help me manage my health and make my life better."

Diabetes and heart failure are a growing epidemic among seniors in the United States, and 99 percent of every dollar spent on Medicare is used to treat these and other chronic conditions. XLHealth's unique business model is focused on improving quality of care and reducing the impact of chronic illness on Medicare beneficiaries and those who care for them.

Care management services like Care Improvement Plus' HouseCalls program allow for direct assessment and coordination of a member's health needs. Just recently, XLHealth expanded its HouseCalls program—now providing members with free in-home doctor or nurse practitioner visits directly following their discharge from the hospital.

"We understand the issues that seniors face when living with chronic conditions," said Paul Serini, executive vice president for XLHealth. "We've been managing the care of those with heart failure and diabetes for a long time and believe that our members benefit tremendously from the support we provide—hitting 100,000 members this month is a testament to that."

Care Improvement Plus launched in 2006 and currently serves seniors in Arkansas, Georgia, Missouri, South Carolina, Texas and eleven Maryland counties.  XLHealth estimates that approximately seven million Medicare beneficiaries across all six states are eligible to enroll in a Care Improvement Plus plan and benefit from its unique model of care.
