100,000 Medicare Beneficiaries Discover Benefits of Special Needs Plans

BALTIMORE, Feb. 1, 2011 /PRNewswire/ -- Today, XLHealth, a leading provider of health plan solutions for Medicare beneficiaries with special needs, announced the enrollment of its 100,000th member, making Care Improvement Plus – the company's chronic condition Special Needs Plan – the largest in the country.

Care Improvement Plus' chronic condition special needs plan combines medical coverage – hospital and doctors' visits – with tailored Medicare Part D prescription drug coverage and care management support for Medicare beneficiaries with diabetes and heart failure.

Care Improvement Plus members and their caregivers are supported under the plan's model of care, which coordinates care across numerous providers and settings. The plan's growth demonstrates that chronically ill seniors are increasingly turning to health plans that meet their complex needs and emphasize care coordination, prevention and wellness to improve their quality of life.
