Satellite Health Plan Launches in Santa Clara County

Satellite Health Plan, Inc., a subsidiary of Satellite Healthcare, Inc., will become effective January 1, 2014. Satellite Health Plan (HMO SNP) offers the same coverage as Medicare but with additional benefits, designed for kidney dialysis patients.
The plan is available to Santa Clara County residents who have End Stage Renal Disease (ESRD) and are Medicare eligible. Satellite Health Plan, will begin enrolling members in its new Medicare Advantage plan beginning October 15, 2013.
“We are really pleased to offer the first ever Medicare Health Plan in Northern California designed specifically for Medicare beneficiaries living with kidney failure,” said Gary Cellini, CEO of Satellite Health Plan. “Members of Satellite Health Plan will have access to care managers and pharmacists who will help them and their families navigate the often confusing health care environment, ensure timely access to care, and coordinate important transitions of care like the one experienced between hospital discharge and arriving at home.”
Under Satellite Health Plan, a team of experts will develop individual care plans to address each member’s unique health needs. The member will be matched with a nurse care manager knowledgeable in ESRD to help them understand their coverage and answer questions.
