, a subsidiary of health and well-being company (NYSE: HUM), has achieved certification by the (NCQA) as a level one Physician Practice Connections – Patient-Centered Medical Home™ (PPC-PCMH), recognizing the company’s long-time commitment to high quality primary and preventive patient care at its medical centers.

The Patient-Centered Medical Home concept has proven a successful model for improving the quality of primary care, while seeking to transform medicine by lowering costs and increasing patient satisfaction.

At the helm are primary care doctors who demonstrate they coordinate all patient care with a team of health care providers, who systematically track tests and results to better identify patients at risk, and who provide prompt access to care and follow up after visits.

“By doing all of this, over a two-year period we’ve reduced unnecessary emergency room visits by 57 percent and in-patient admissions by 42 percent across patients in our Diabetes program,” said CAC-Florida Medical Centers CEO Mark Kent. “The medical home concept, with a focus on primary and preventive care, has always been at the heart of our approach to quality patient care. We’re proud that our medical centers and our primary care physicians have achieved the rigorous standards that physician practices must meet in order to attain this significant accomplishment.”

All CAC’s medical centers, except for its three newest centers, one in Liberty City and two in Central Florida, have achieved the PPC-PCMH quality designation.

Kent said the Patient-Centered Medical Home is also serving to invigorate the role of primary care physicians who are needed to serve the country’s booming senior population, many of whom see multiple doctors and take a mix of prescriptions.

CAC-Florida Medical Centers also were previously awarded three-year recognition through the Diabetes Recognition Program developed by the NCQA and the American Diabetes Association to recognize physicians who have an established track record of providing excellent diabetes care to patients.
