ActiveHealth Management Health Informatics Executive to Discuss the Role of Clinical Analytics in Supporting a Statewide M

NEW YORK--(BUSINESS WIRE)-- ActiveHealth Management (ActiveHealth®) Senior Vice President Iver Juster, M.D., will speak at the Forum 11 on Sept. 8, 2011 to share how ActiveHealth is using its broad experience in clinical analytics and care management to support and evaluate a North Carolina statewide medical home model and population health management program. Dr. Juster will present “An Evaluation Framework for a Statewide Medical Home Care Delivery Collaborative,” along with Anne B. Rogers, RN, BSN, MPH, the director of Integrated Health Management for the State Health Plan for Teachers and State Employees in North Carolina, and L. Allen Dobson, M.D., president and CEO for Community Care of North Carolina (CCNC). The North Carolina State Health Plan for Teachers and State Employees (“the Plan”) is working with ActiveHealth to implement a new patient-centered medical home (PCMH) model for its 548,000 members beginning later this fall.

“North Carolina is pioneering one of the largest moves toward a community-based medical home model of care for its members, with the goal to improve members’ health. More than half of the Plan’s members have a chronic disease, and those individuals drive about 76 percent of the Plan’s health costs,” Dr. Juster explained. ActiveHealth will provide the physicians and health care providers with clinical insight, actionable analytics and work flow tools – both at the individual patient and population level – to help them improve the health of the Plan’s members.

At Forum 11, Dr. Juster will share the care coordination approach and the framework ActiveHealth developed to evaluate the impact of The State Plan’s medical home model and care management programs. “This framework will provide important guidance for others that are looking to implement similar approaches to improve health care outcomes and reduce the costs of care,” Dr. Juster said.
