, a market leader in ambulatory clinical systems, today announces that New York’s is expanding its relationship with the company to include eClinicalWorks Care Coordination Medical Record™ (CCMR) for advancing its -related objectives, coordinating care across the continuum as well as evaluating population health and quality improvement activities. UHP’s more than 100 medical providers currently utilize eClinicalWorks comprehensive electronic health records (EHR) solution and the organization received a 2009 HIMSS Davies Award for its EHR implementation.

“As a Federally-Qualified Health Center, Urban Health Plan works diligently to eliminate healthcare disparities in our communities,” said Paloma Hernandez, president and CEO, Urban Health Plan, Inc. “During the past few years, we have changed our delivery of care and have been able to track healthcare outcomes. We look forward to continuing to work with eClinicalWorks on improving the health of the areas we serve, namely the South Bronx and Corona, Queens.”

The ACO care delivery model coordinates care between patients and members of the healthcare team with the goal being to help strengthen the physician-patient relationship, leading to better care and reduced avoidable costs over time. eClinicalWorks CCMR is designed to provide visibility into a patient’s care across all settings, facilitate smooth transitions of care, engage patients and providers in preventative care, improve outcomes among populations and reduce costs. A vendor-neutral, open network securely connects ambulatory EHRs with hospital in-patient systems and payor claim feeds, including Centers for Medicare & Medicaid Services (CMS), to provide cost and utilization for services delivered to patients.

Included in CCMR, eClinicalWorks Community Analytics™ uses closed-loop analytics to deliver reporting, alerting and messaging capabilities necessary to manage Population Health. Dashboards can be shown at the community level to determine and monitor adherence of clinical process and outcomes, for including medication recalls and chronic disease management. Care coordination and analytics work together for a collaborative care framework.

“Urban Health Plan has long been an early proponent of using technologies that will ultimately help its patients,” said Girish Kumar Navani, CEO and co-founder of eClinicalWorks. “eClinicalWorks CCMR will allow care coordination from a variety of environments, giving organizations the tools they need to effectively undertake comprehensive care delivery. As demonstrated with the New York City Department of Health & Mental Hygiene, our closed-loop analytics enhances decision-making at the point of care. We are excited to expand our relationship with UHP and aid the organization in its care delivery goals.”
