Medinova Physicians Partners with PatientPoint to Improve NCQA Rating and Streamline Care Coordination Processes

PatientPoint will automate management of clinical reporting needs, helping group to achieve NCQA Level 3 Patient-Centered Medical Home Certification

ORLANDO, Fla.--(BUSINESS WIRE)-- PatientPoint™, a provider of care coordination and revenue cycle management solutions, today announced that Medinova Physicians LLC has expanded its existing partnership with the company to include care coordination technology and National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) consulting services.

The Bronx, NY-based, five location practice specializes in family and internal medicine. Medinova selected PatientPoint to assist in its mission of achieving Level 3 PCMH recognition, which is the highest NCQA designation.

The practice will deploy PatientPoint’s HealthSync Care Coordination Platform, which will integrate with its existing practice management and electronic health records (EHR) systems. The platform automates care coordination processes and simplifies patient engagement through the following:

  • Identifying gaps in care and formulating care plans for patient adherence by analyzing patient self-reported data, existing patient records and information from health plans;
  • Connecting patients with valuable care-related information pre- and post-service via online or mobile outreach, as well as at the point of care;
  • Providing point-of-service clinical decision support and compliance reminders to providers based on each patient’s history; and
  • Streamlining clinical review, quality reporting and performance measurements to more easily meet certification standards.

By leveraging this approach, Medinova will be able to better satisfy PCMH requirements and move from its current Level 1 PCMH recognition to Level 3. For example, Medinova will be able to identify and address care gaps for patients with chronic conditions such as hyperlipidemia, hypertension and diabetes through an automated process. The practice will then monitor and report on this population health information and related improvements—a key component to NCQA certification.

“Selecting PatientPoint as we embark on this mission was an easy choice,” said Ventkata Ravi, M.D., president of Medinova Physicians. “Their in-depth expertise and years of experience in consulting on quality care initiatives—paired with a keen understanding of physician office workflow and design related to the PCMH model—were the primary drivers in our decision. With PatientPoint’s technology we can directly satisfy most of the NCQA care coordination requirements through automation, enabling us to focus more on the care we provide.”

“The partnership with Medinova illustrates our commitment to helping practices meet and maintain Level 3 PCMH compliance,” said Raj Toleti, CEO of Patient Point. “We embrace the opportunity to work with Medinova as we guide them through the certification process, and look forward to providing them with the tools needed to fully operationalize the administrative tasks associated with care coordination and patient engagement.”

Medinova originally began working with PatientPoint in 2011 to launch a patient portal and electronic point-of-service check-in solution. The primary focus of the deployment was to improve administrative efficiency associated with pre-registration and registration, which included demographic capture, eligibility and benefits verification, clinical form intake with e-signature and bill pay for co-payments and collections.
