Jiva, ZeOmega’s Integrated Care Management Solution, Embeds Healthwise Decision Support

Health Plans Can Now Leverage Healthwise Engagement Tools From Within Jiva

FRISCO, Texas & BOISE, Idaho--(BUSINESS WIRE)-- ZeOmega and Healthwise announced today an agreement to integrate Healthwise® product offerings with ZeOmega’s integrated care management solution, Jiva. The integration allows health plans to access Healthwise® Knowledgebase, Healthwise® Care Support Pages, Healthwise® Ix® Patient Instructions, Healthwise® Ix® HealthMastery® Campaigns, and Healthwise® Ix® Conversations from within Jiva—thereby streamlining and automating the delivery of this important content to its members, physicians, and the care community.

Jiva, a web-based integrated care management platform, transforms traditional episodic-based care management into proactive and collaborative population healthcare management through electronic health record-enabled (EHR-enabled) care plans, point-of-care tools, decision support, and user-configurable workflows for care management.

Health plans using Jiva will have access to a robust resource of Healthwise information and tools, including decision aids and thousands of Knowledgebase topics on chronic conditions, wellness, medical tests, procedures, and medicines; patient instructional health topics covering after-visit, preoperative, and post-operative categories; condition-specific chronic health information pages; and wellness campaigns supported by multimedia tools—all of which provide members with in-depth, action-focused knowledge around teachable moments.

“Tight integration with Healthwise’s offerings takes us one step closer to revolutionizing population healthcare management by ensuring that members, care managers, providers, and caregivers have access to relevant health educational information at the right time,” said Sam Rangaswamy, ZeOmega’s Founder and Chief Executive Officer. “Jiva’s rules packages for Healthwise content streamline and automate the delivery of such content based on the consumer’s channel preferences at the right time. Our clients are able to maximize the impact of such communications, thereby positively influencing member behavior.”

“If you want more engaged consumers, give them the tools they need to make smarter treatment decisions with their doctors and help them do more for themselves at home,” said Don Kemper, Healthwise CEO. “ZeOmega and Healthwise are working together to get people the specific information tools they need at the time they need them.”
