Care Team Connect to Coordinate Community-Wide Effort for Patient-Centered Medical Home

Expands Proven Model to 50,000 Patients in Calhoun County, Mich.

EVANSTON, Ill. & BATTLE CREEK, Mich.--(BUSINESS WIRE)-- Officials with Care Team Connect announced today the expansion of their partnership with Integrated Health Partners (IHP) to 50,000 patients across Calhoun County, Michigan as part of the Medicare-sponsored Michigan Primary Care Transformation Demonstration Project.

IHP currently utilizes Care Team Connect’s industry-leading care coordination platform as the backbone for the Blue Cross Blue Shield of Michigan Provider Delivered Care Management pilot. Through the pilot, Care Team Connect and IHP have improved care management of a select population of patients and proven the scalability of Care Team Connect’s robust, easy-to-use platform.

IHP will continue to grow their Patient-Centered Medical Home initiative with Care Team Connect over a three-year period commencing January 1, 2012. During this time, IHP will further demonstrate that their care delivery model reduces costs and improves quality by offering critical improvements in the coordination of patient-specific care teams through increased care manager efficiency, pragmatic clinical resource utilization, targeted patient activation and family participation, robust community resource engagement and measurement of referral effectiveness.

“All along, our goal has been to significantly improve care in the communities we serve by making it more affordable and accessible to our patients, while at the same time lending support to our physicians in the difficult task of transforming care delivery,” said Ruth Clark, Executive Director of IHP. “Care Team Connect helped us establish and share our best practices initially, and will now support us as we look to scale these best practices across more providers and, most importantly, more patients.”

The initiative targets patients with multiple payers, including the expansion of IHP’s existing work with Blue Cross Blue Shield of Michigan, as well as Blue Care Network, Health Plan of Michigan, Medicare and Medicaid managed care plans. The primary components include the large volume of patients, supportive network of physician organizations, PCMH transformation process and the care management component.

By using Care Team Connect, hospitals, community providers, family members and patients can become more coordinated and ultimately improve patient outcomes, while significantly reducing healthcare costs. Based on the unique needs of each patient, Care Team Connect generates a longitudinal and patient-specific care plan rooted in best practices that all care providers can share and follow, facilitating increased patient engagement, as well as efficient access to information across time and care setting.

“The structure of sustainable care delivery in this country is shifting toward highly efficient and well-coordinated care, and we are pleased to extend our partnership with a visionary organization like IHP who continues to focus on building processes that better support the community of Battle Creek,” said Ben Albert, CEO and co-founder of Care Team Connect. “We applaud their effort and are proud that IHP has chosen to leverage our platform to deliver efficient, high quality and affordable care to thousands of patients.”
