Care Team Connect to Coordinate Community-Wide Effort for Patient-Centered Medical Home

Expands Proven Model to 50,000 Patients in Calhoun County, Mich.

EVANSTON, Ill. & BATTLE CREEK, Mich.--(BUSINESS WIRE)-- Officials with Care Team Connect announced today the expansion of their partnership with Integrated Health Partners (IHP) to 50,000 patients across Calhoun County, Michigan as part of the Medicare-sponsored Michigan Primary Care Transformation Demonstration Project.

IHP currently utilizes Care Team Connect’s industry-leading care coordination platform as the backbone for the Blue Cross Blue Shield of Michigan Provider Delivered Care Management pilot. Through the pilot, Care Team Connect and IHP have improved care management of a select population of patients and proven the scalability of Care Team Connect’s robust, easy-to-use platform.

IHP will continue to grow their Patient-Centered Medical Home initiative with Care Team Connect over a three-year period commencing January 1, 2012. During this time, IHP will further demonstrate that their care delivery model reduces costs and improves quality by offering critical improvements in the coordination of patient-specific care teams through increased care manager efficiency, pragmatic clinical resource utilization, targeted patient activation and family participation, robust community resource engagement and measurement of referral effectiveness.

The primary components include the large volume of patients, supportive network of physician organizations, PCMH transformation process and the care management component.

By using Care Team Connect, hospitals, community providers, family members and patients can become more coordinated and ultimately improve patient outcomes, while significantly reducing healthcare costs.
