Study Shows Univita Health’s New Re-admission Reduction Solution Reduces Hospital Re-Admissions among Chronically Ill Patients

, the nationwide leader in post-acute and home healthcare solutions, just completed a study on its new high-quality Re-admission Reduction Solution. The comprehensive initiative provides a truly integrated approach to chronic disease management aimed at keeping patients healthy and in the comfort of their homes, while preventing costly and unnecessary re-hospitalizations.
The holistic program is administered by specially-trained registered nurses and is centered on the patient, caregiver and primary care physician (PCP). The study shows this approach reduces hospital re-admissions down to 8.6% among patients with conditions such as: chronic heart failure (CHF), chronic obstructive pulmonary disease (COPD), diabetes, hypertension and chronic kidney disease.

The findings show that prior to implementing the Univita Health Re-admission Reduction Solution, patients whose records were analyzed over nine months were being re-admitted to the hospital at a rate of 53%. After program completion, the same patients were being re-admitted at a rate of 8.6%.
