New technology platform uses evidence-based best practices to reduce preventable readmissions

CTC is a web-based, secure platform that enables post-discharge care coordination across hospitals, physician practices, community providers, families and patients. Today, one in five Medicare patients is readmitted to the hospital within 30 days of discharge. Seventy-five percent of these admissions are preventable, costing Medicare approximately $12 billion per year. In response, the Centers for Medicare and Medicaid Services (CMS) plans to generate $26 billion in cost savings over the next decade by making hospitals more accountable for their readmissions through publishing hospital readmission rates and financially penalizing poor performance. A majority of these readmissions already represent a significant financial loss for hospitals. The pending financial penalties have created an urgent need for hospitals to better control costs through better post-discharge care coordination.
