SKOKIE, Ill.--(BUSINESS WIRE)-- Remcare, the care team coordination and software-as-a-service leader, officially announces the release of its latest product, Care Team Connect (CTC). Today, one in five Medicare patients is readmitted to the hospital within 30 days of discharge. Seventy-five percent of these admissions are preventable, costing Medicare approximately $12 billion per year. In response, the Centers for Medicare and Medicaid Services (CMS) plans to generate $26 billion in cost savings over the next decade by making hospitals more accountable for their readmissions through publishing hospital readmission rates and financially penalizing poor performance. A majority of these readmissions already represent a significant financial loss for hospitals. The pending financial penalties have created an urgent need for hospitals to better control costs through better post-discharge care coordination.
