Remcare’s Care Team Connect Breaks Readmission Cycle

New technology platform uses evidence-based best practices to reduce preventable readmissions

SKOKIE, Ill.--(BUSINESS WIRE)-- Remcare, the care team coordination and software-as-a-service leader, officially announces the release of its latest product, Care Team Connect (CTC). CTC is a web-based, secure platform that enables post-discharge care coordination across hospitals, physician practices, community providers, families and patients. It combines patient centric and evidence-based models of care to provide clear information to everyone on the healthcare team about the patient’s risk of readmission, what care is needed, when it is needed, and who is responsible for monitoring the plan.

Today, one in five Medicare patients is readmitted to the hospital within 30 days of discharge. Seventy-five percent of these admissions are preventable, costing Medicare approximately $12 billion per year. In response, the Centers for Medicare and Medicaid Services (CMS) plans to generate $26 billion in cost savings over the next decade by making hospitals more accountable for their readmissions through publishing hospital readmission rates and financially penalizing poor performance. A majority of these readmissions already represent a significant financial loss for hospitals. The pending financial penalties have created an urgent need for hospitals to better control costs through better post-discharge care coordination.

Health care organizations have identified CTC as the solution for superior risk stratification and care team coordination upon discharge from the hospital. CTC uniquely provides both clinical and financial value, driving better quality care and patient outcomes as well as millions of dollars of cost savings for hospitals by reducing preventable readmissions.

“In over 25 years of nursing the root cause of readmissions has always been obvious – patients with poor self management skills, lack of a common platform for care team communication and coordination, and inability to predict which patients need what level of support,” states Jane Ruhl, RN, Care Manager for Integrated Health Partners. “CTC is a revolutionary tool for care managers and community providers as we work together to solve a long-standing and expensive problem.”

CTC enables hospitals, accountable care organizations, and community care managers to reduce the cost of care for patients both in and out of the hospital while improving the quality of care provided. The results at CTC pilot sites in Napa, California and Battle Creek, Michigan illustrate CTC’s impact on making readmission prevention efforts sustainable on a large scale with minimal resources.

Remcare co-founder and CEO, Ben Albert, notes that CTC will lead to significant return on investment in both real dollars and patient outcomes.

“CTC represents a new way of thinking about how we deliver post-discharge care in the community,” states Albert. “Our team is staffed with experienced clinicians, operational expertise, and technology leaders. We leverage their collective knowledge and our clients’ thought leadership in a collaborative effort to cross-pollinate evidence-based best practices. This has proved invaluable as our clients yield the best results with their available resources.”
