The Centers for Medicare & Medicaid Services has added 17 hospitals and community-based organizations to participate in the Community-based Care Transitions Program, the agency announced Friday. With these new sites, the number of providers participating in the effort to cut down on hospital readmissions now totals 200. The newest additions include New York City's Mt.
