HealthWyse And Cardiocom Partner To Create More Robust Clinical Records For Home Care Clinicians

Companies integrate telemonitoring data with EMR system to capture vital patient health information

WILMINGTON, Mass.--(BUSINESS WIRE)-- HealthWyse, a leading provider of home health and hospice software and services, announced today that it has partnered with Cardiocom, a leader in home telehealth solutions, to seamlessly integrate patient information into HealthWyse’s clinical software.

“HealthWyse has always been focused on addressing the specific needs of home health and hospice clinicians,” said Mike Kramer, Ph.D., CEO of HealthWyse. “Together with Cardiocom, we’re making sure home care nurses and clinicians have all of the necessary patient information contained in the HealthWyse electronic medical record to enable them to focus on the patient and deliver quality care.”

This software integration enables home health nurses and clinicians to have continuously updated information on patients suffering from chronic conditions including congestive heart failure (CHF) and chronic obstructive pulmonary disease (COPD), among others. Cardiocom’s telemonitoring services track vital signs, symptoms data and other important information to help patients and their clinicians monitor their condition. By integrating this information into HealthWyse, home care clinicians have access to all of the patient’s medical information at the point of care and in turn can make a positive impact on patient care.

"Cardiocom is committed to working with enterprise level home care technology firms to allow telehealth data to follow the patient throughout the care continuum,” said Daniel L. Cosentino, CEO, and President of Cardiocom. “We are excited about the benefits that this new relationship provides to our clients."

HealthWyse customers are already seeing the benefits of the new software integration. The Norwell Visiting Nurse Association (NVNA) and Hospice, in Norwell, Mass., monitors nearly 130 patients using Cardiocom systems and immediately recognized the value of integrating Cardiocom data into HealthWyse. The data is available in real-time, providing nurses and clinicians in the field with a more complete patient record with which to make workflow and care decisions. As a result, caretakers can swiftly intervene in the event of an emergency, provide additional care when needed and deliver better patient care overall.

“By combining the telemonitoring information with the clinical chart, our nurses are armed with a complete record to provide better patient care and avoid hospital readmissions,” said Jackie Crossen-Sills, PT, Ph.D., Director of Education & Research at NVNA and Hospice. “This functionality lets us make real time decisions that result in better patient outcomes.”
