The prestigious certification demonstrates compliance with key standards for NCQA health plan accreditation, including high standards of disease management care for people with asthma, diabetes, chronic obstructive pulmonary disease (COPD), heart failure and ischemic vascular disease (IVD). Health Dialog’s Disease Management program has been recognized by NCQA since 2002.
