Dovetail Health Supports Newly Formed Accountable Care Organizations by Helping to Reduce Unnecessary Hospital Admissions

NEEDHAM, Mass.--(BUSINESS WIRE)-- Today, Dovetail Health has extended its innovative Complex Care Management and Transition Management programs to support the unique needs of newly forming Accountable Care Organizations. Dovetail’s programs are focused on supporting the highest risk patients in order to prevent unnecessary hospital admissions and readmissions.

Relatively few patients, approximately 10%, account for nearly 70% of all healthcare expenditures. These are the most complex patients, dealing with multiple chronic conditions, multiple prescription medications and a host of other complications that in combination lead to frequent and costly utilization. Dovetail’s Complex Care Management program uses predictive modeling to proactively identify high risk patients and deploys an interdisciplinary team of nurses, pharmacists and social workers to prevent unnecessary hospitalizations and emergency room visits.

On average, 1 in 5 seniors are readmitted to the hospital within 30-days of being discharged, at a cost of over $20 billion per year. Complicated medication regimens are particularly problematic, contributing to as many as two-thirds of adverse events post-discharge. Dovetail’s Transition Management Program deploys specially trained Pharmacist Care Managers to meet patients in their homes immediately following discharge to conduct a thorough assessment of medications and other readmission risk factors.
