When CCHP determined their existing care management system was not meeting their needs, they knew they had to find a solution that could be implemented cost effectively. Their solution would also have to meet the state's requirement for a 24-hour Nurse Advice Line, and the lead time for the implementation was short.

CCHP chose DST Health Solutions' Care Analyzer and CareConnect, the foundations of DST's Integrated Care Management (ICM) suite of solutions, which is integrated in real time with CCHP's claims processing system. ICM identifies members most in need of care, allowing CCHP to intervene and provide enhanced care management; following up with systematic evaluations to ensure all members receive the proactive care they need.

There was also the issue of timing. CCHP had a state-mandated deadline for the Nurse Advice Line, plus an internal deadline for getting the ICM solution up and running.
