Blessing Health System Advancing Quality Patient Care with Rothman Healthcare Corporation’s Patient Condition Tracker

SAN FRANCISCO--(BUSINESS WIRE)-- Blessing Health System announces a partnership with Rothman Healthcare Corporation to implement the Patient Condition Tracker within the Allscripts Sunrise Clinical Manager electronic health record used at Blessing Hospital and Illini Community Hospital. Additionally, Blessing-Rieman College of Nursing will be a major participant in research studies regarding the Patient Condition Tracker.

“Blessing Hospital and the Blessing-Rieman College of Nursing are natural partners given our shared focus to increase the continuity and quality of care that patients receive nationwide,” says Rothman CMO, Dr. Duncan Finlay. “Our first trial at Sarasota Memorial Hospital demonstrated that our tracker leads to improved discharge dispositions and better continuity of care. We’re looking forward to demonstrating that at Blessing.”

Since 2004, Rothman Healthcare Corporation has developed the Patient Condition Tracker to bridge the gaps in continuity of care. The Patient Condition Tracker uses electronic health record (EHR) data to display the progression of a patient’s health. The Patient Condition Tracker generates a regularly updated health score, based on vital signs, nursing assessments, and lab results, in a user-friendly graphical format, effectively summarizing thousands of pages of patient data at a glance. This makes it easy to track patient progress and detect subtle declines in health. There also is the ability to see multiple patient graphs simultaneously for a summarized understanding of a whole unit, helping doctors, nurses, and Rapid Response teams get patients the care they need as effectively as possible.

“We are excited to be a part of this innovative approach to improving patient care using advanced technologies used at Blessing and Illini. The Patient Condition Tracker epitomizes meaningful use of the Electronic Health Record, allowing us to stay at the forefront of quality patient care,” says Dr. Debra Phillips, Blessing CMIO. “In addition to the installation of the Patient Condition Tracker at the hospitals, we are creating a research alliance between Rothman Healthcare Corporation and the Blessing-Rieman College of Nursing.”
