An independent panel of judges ranked entries from national, state and local health-related organizations based on user experience, content, design and creativity.
MyActiveHealth offers members personalized resources to help them take action to improve their health. - Secure messaging so that members can communicate with their Disease Management nurse coaches, making it easy to stay connected between appointments. Nurse coaches can send follow-up items to members and assign homework to be completed on MyActiveHealth.

Members can use online coaching tools to help manage chronic conditions and change lifestyle behaviors.
