Assort Health rolls out AI agent to automate referrals for specialty practices and health systems

Artificial intelligence platform Assort Health launched a new AI agent to aid healthcare organizations in handling referrals.

The agent—aptly dubbed Referrals—has a range of capabilities, including processing inbound referrals, confirming eligibility and matching patients to best-suited providers. The tool ingests referrals from sources such as electronic health records, faxes, PDFs and phone calls, verifies eligibility, matches patients with the most appropriate provider.

Assort Health Founder and Co-CEO Jon Wang told Fierce Healthcare the AI agent identifies a better-fit provider than the original in 50% of referrals. “This saves providers from seeing the wrong patients, and gets patients seen faster by the right one,” Wang said.

Moreover, according to Wang, the tool sees a 79% referral-to-appointment conversion rate and 87% of all referrals are fully automated end-to-end.

The referrals agent can also proactively reach out to patients via voice, text or email to book appointments directly in the EHR and enable continuous patient conversations.

The agent is driven by Synapse, Assort’s proprietary model built on more than 225 million specialty-specific patient interactions, 70,000 care protocols and decision pathways, according to Jeffery Liu, Assort Health founder and co-CEO.

“I think that's a huge differentiation for our platform,” Liu said. “Other products, other solutions, they don't have the level of personalization to really target patients the way that we do.” 

Wang said Assort’s suite of products creates a “unified patient journey.”

“We're able to handle interactions seamlessly and create that very, very unified personalized interaction,” Wang said.

One of the practices using Referrals is Chattanooga, Tenn.-based Center for Sports Medicine & Orthopaedics.

Call Center Manager Jensine Joseph said in a statement the referral team was previously “overwhelmed by the volume of incoming referrals.” Joseph added initial outreach could take “three or four business days” and follow-ups sometimes “didn't happen for a week or more.”

“Today, Assort automatically processes referrals, verifies key information, and proactively reaches out to patients within hours of receiving the referral,” Joseph said. “That speed has made a tremendous difference. Patients hear from us before they start looking for other options, and we're now automating about 90% of referrals end-to-end and seeing a 77% conversion rate.”

Assort Health has seen a plethora of expansion in 2026, including the launch of Activate for proactive patient outbound calling in May. It works with “several hundred customers” now, Wang said.

In June, the startup also announced a $120 million series C funding round—bringing its total valuation to $1.2 billion.
