"residents are not machines"...right on! and so with nurses (heparin mistake?)even computers start malfunctioning after it has been on for some time. continuity of care can always be discussed during endorsement. Let us all please recognize human limitations.

Doctors, Residents, Nurses and their patients too - all are subject to their own biorythms. The irony of this situation is serious business. Patients deserve better also!
