Nicole Harris-Hollingsworth

Nicole Harris-Hollingsworth, EdD, MCHES

Dr. Nicole Harris-Hollingsworth, EdD, MCHES, a senior health systems scientist, is the Vice President, Social Determinants of Health for Hackensack Meridian Health in New Jersey.

Dr. Harris-Hollingsworth leads the development and implementation of network wide social determinants of health strategy, stakeholder engagement interventions, and the development of community accessible resources across the Hackensack Meridian Health network. 

For over two decades she has specialized in the development of community health programs designed to create behavior change and increase knowledge in hard to reach populations including regional school-based programs, national reproductive health organizations and national child welfare organizations.  Her interest areas include the development of effective regional collaborations to increase health equity and increasing the use of shared decision making practices between health care consumers and health care providers to facilitate community engaged research.

Gillian Feldmeth

Gillian Feldmeth

Gillian Feldmeth is the Director of Research and Evaluation Operations at Unite Us. In this role, she supports development and execution of the organization’s strategic framework to contribute to the industry’s growing knowledge about the real-world impact of investing in social care. She comes to Unite Us with nearly 10 years of experience in implementing and evaluating technology-enabled interventions designed to improve health and reduce disparities. Prior to joining Unite Us, Gillian led insights and evaluation at NowPow (now a Unite Us company), led special projects in a University of Chicago research lab and served as a patient educator for women recovering from cancer. Her previous research experience includes health systems’ role in addressing social drivers of health, novel recruitment methods to improve diversity in clinical trials and improving sexual function outcomes for women with cancer.

Gillian holds a Bachelor’s of Science degree in Social Policy, Public Health and Political Science from Northwestern University.

Angela Schubert

Angela Schubert

Angela Schubert, MPH, serves as a Senior Analyst at BJC HealthCare Community Health Improvement, where she brings a wealth of expertise and dedication to her role. As a seasoned professional committed to data-driven decision-making, Angela plays a crucial role in evaluating and enhancing community health programs. Her work ensures that these initiatives are not only effective but also responsive to the diverse needs of the populations they serve. Angela’s passion for healthcare innovation and addressing social determinants of health (SDOH) within the healthcare ecosystem has resulted in groundbreaking improvements to SDOH documentation and data utilization within hospital systems and the broader community through the St. Louis Community Information Exchange. Prior to joining BJC, Angela made significant contributions to public health at the Missouri State Health Department, where she focused on validating ICD-10 code utilization and diagnostic outcomes related to Zika and Neonatal Abstinence Syndrome.
