A new infographic highlighting the state of chronic disease of older Americans and how healthcare providers can more effectively and efficiently care for these conditions was issued today by . It can be accessed at: .

According to the infographic the record number of more than 49 million Medicare beneficiaries in the United States is the result of a combination of longer life spans and the aging of the baby boomer generation. The healthcare challenge that accompanies these numbers is the fact that two-thirds of Medicare beneficiaries have two or more chronic conditions, and $3 out of every $4 spent on healthcare is the result of these ailments. That amounts to nearly $7,900 for every American with a chronic disease. The five most common chronic conditions are high blood pressure, high cholesterol, heart disease, arthritis and diabetes.

As the infographic reveals Clear Vision’s RevMaxPro product helps to ensure that these conditions are identified and properly treated and that providers and health plans can expect to receive reimbursement from the Centers of Medicare and Medicaid (CMS) that is commensurate with the cost of delivering this care. Clear Vision clients see an average 18 percent increase to their risk adjustment factor in year one and an average increase of 56 percent in year two.

Launched in August RevMaxPro is a second generation of one of the healthcare industry’s most powerful and easy-to-use data-intelligence tools that helps physicians and health plans better care for Medicare patients while optimizing their own revenue potential. RevMaxPro helps identify patients with chronic conditions, pinpoint gaps in treatment, creates patient evaluation forms to ensure chronic conditions are addressed, monitors the reporting of clinical data to CMS, and aligns revenue with expected costs of care.

“RevMaxPro is integral to effective care management, finance and CMS compliance because it helps ensure that all of a patient’s chronic care needs are being identified and properly addressed based on real-time data that physicians readily have at their fingertips,” says Tom Peterson, president and CEO of Clear Vision. “In doing so RevMaxPro allows physicians to quickly and easily make the connection between services delivered and CMS revenue received.”
