EHRs can help reduce readmissions, study shows

Electronic health records can help predict which inpatients are at high risk for readmission, and do so in real time, according to a new study in BMC Medical Informatics and Decision Making. Not just hospitals, but also physician practices have increasingly been pressed to improve patient outcomes by reducing the readmission rates of discharged patients. Authors say that methods to identify those at the highest risk of adverse events allow for sufficient time to initiate and coordinate the concentration of scarce resources on those most likely to benefit.  And organizations will then have greater potential to accomplish the "Triple Aim" goals of higher quality, more cost-conscious care for patients and populations. >> Read the full article from FierceEMR
