SPOTLIGHT: Illinois Blues targets hospital readmission rates

Blue Cross and Blue Shield of Illinois and the Illinois Hospital Association are partnering on a venture to improve patient quality via care transition, palliative care improvements and reduced readmission rates, reports FierceHealthcare. The two groups are aiming to reduce hospital readmissions by 33 percent by 2014.
