Out-of-network coverage missing from most New York exchange plans

Even though New York's insurance exchange offers consumers hundreds of plans, most residents can't figure out if the plans offer out-of-network coverage, reported Kaiser Health News. That's because they usually don't--throughout most of the state, the only plans available are health maintenance organization plans that cover care only for providers in the plan's network. Some experts say this is the result of preferred provider organizations; these plans usually cover some out-of-network providers but leave customers to pay the bills, since deductibles and co-insurance charges are higher.
