Intercede Health Announces Plans for Comprehensive Diabetes Care Clinics

Clinics will offer a one-stop shop for diabetes education, treatment, and management

HOUSTON--(BUSINESS WIRE)-- Intercede Health, a privately held medical management company founded in 1996, announces plans to open comprehensive diabetes care clinics in the greater Houston area.

Current statistics show that over 18.8 million people in the US have a diagnosis of diabetes, and an estimated additional 7 million people have undiagnosed diabetes. “Everyone realizes that diabetes is one of the real health crises of modern times,” says Intercede Health Chief Medical Officer Joseph Jordan. “Our goal is to provide comprehensive diabetes care—physician care, education, nutrition and diet counseling, lab services, and eye exams—in a way that is convenient and cost-effective for patients in order to increase compliance with care plans and improve patient outcomes.”

Intercede Health currently develops and manages hospitalist programs, provides medical management consulting and physician advisor services for health plans, hospitals and accountable care organizations, and operates outpatient specialty clinics designed to prevent avoidable admissions and readmissions in high-risk patient populations. The company’s existing outpatient clinics—called Transitional Care Centers—treat complicated, complex diabetics as well as patients with COPD, heart failure, coronary artery disease and other chronic conditions in high-risk populations like Medicare and Special Needs Plans. The planned diabetic clinics expand the scope of patient services to a broader population (including commercial and employer plan populations) while focusing exclusively on diabetes and diabetes-associated conditions.
