PatientPoint will automate care coordination processes, helping physician groups to achieve National Committee for Quality Assurance (NCQA) Level 3 accreditation

ORLANDO, Fla.--(BUSINESS WIRE)-- PatientPoint™, a provider of care coordination, patient engagement and revenue cycle management solutions, today announced that Forest Hill Family Health Associates has expanded its existing partnership with the company to include care coordination services and solutions.

Forest Hill, a comprehensive primary care center based in Newark, N.J., is an NCQA recognized Patient Centered Medical Home, which is already part of Horizon Healthcare Innovations, Inc. Patient Centered Medical Home payment model reform initiative. The practice was recently selected by Horizon NJ Health, a Medicaid managed care organization, as the first practice to pilot its Medicaid Patient Centered Medical Home Initiative.

This has presented a unique challenge for the practice to manage healthcare for a wide array of plan beneficiaries while catering to a multitude of quality indicators for different payers. As a result, Forest Hill required a uniform way of providing care coordination and quality reporting.

“Becoming an early adopter of these new quality care models across payers presents a great opportunity for family physicians, but it also demands that we examine the most efficient ways to facilitate care coordination across payers so others have an example on how to achieve success,” said Tom Ortiz, M.D., founder and medical director of Forest Hill Family Health Associates and chairman of the board of trustees at the New Jersey Academy of Family Physicians. “PatientPoint’s care coordination technology enables us to automate patient communications where appropriate in addition to care coordination reporting and tracking across payers. Without this support, we must rely on manual processes — something that proves to be too costly and time consuming.”

By leveraging PatientPoint’s care coordination and patient engagement platform, the practice will be able to:

  • address gaps in care by aggregating payer data from Horizon Blue Cross and Blue Shield (BCBS) and Horizon NJ Health, and integrating it into the electronic health record;
  • share the most relevant health content with the provider and patient at the point of service based on individual patient needs, leading to more meaningful patient-provider communications;
  • engage the patient before, during and after care to improve outcomes and adherence to outlined care plans;
  • streamline workflow and increase productivity for those nurses involved in the care coordination process;
  • collect, measure and report on metrics as they relate to the individual physicians, specific populations, clinical and utilization outcomes and quality of care.

As a Horizon BCBS and Horizon NJ Health featured PCMH program site, Forest Hill has a sponsored embedded population care coordinator who is tracking and reporting on HEDIS and other required quality and utilization metrics. To date, the PatientPoint platform has significantly increased the productivity and patient reach of this population care coordinator.

“Dr. Ortiz’s practice is a pioneer and innovator on the forefront of what’s to come in the healthcare industry,” said Raj Toleti, CEO of PatientPoint. “By connecting all key players in healthcare — the provider, patient and health plan — our solutions enable practices and hospitals alike to pursue quality care initiatives like PCMH, without burdening already busy staff.”

Forest Hill has been a partner of PatientPoint since 2010, leveraging patient engagement and revenue cycle management services.
