Clinical Analytics Platform From Optum Supports Accountable Care Initiatives at Mercy and HealthEast Care System

Advanced clinical analytics technologies from Optum are supporting population health management and accountable care strategies at Mercy, the nation’s sixth-largest Catholic health care system, and HealthEast Care System, the largest locally owned health care organization in the Minneapolis-St. Paul east metro area.

HealthEast, which is composed of four hospitals and 1,500 physicians, is using the Humedica analytics platform from Optum to analyze data from a variety of clinical and administrative sources to better understand and manage treatment for patients with chronic conditions, and to advance performance for its newly formed accountable care organization. HealthEast began using Humedica analytics to identify gaps in care, determine which patients are at greatest risk of re-hospitalization or of developing chronic conditions and create strategies for managing or coordinating care for high-utilizers.

At Mercy, the Humedica clinical analytics platform supports the health care system’s accountable care initiatives.
