Empire BlueCross BlueShield is a Health plan with a Medicare contract. Empire HealthChoice HMO, Inc. (Empire), an independent licensee of the Blue Cross and Blue Shield Association, is the legal entity that has contracted with the Centers for Medicare & Medicaid Services (CMS) to offer the Empire MediBlue HMO plans. Empire is the risk-bearing entity licensed under applicable state law to offer the Empire MediBlue HMO plans. Empire has contracted with CareMore to provide covered services under the Empire MediBlue HMO plans. CareMore Care Centers are available to Medicare beneficiaries who enroll in select Anthem HealthKeepers Medicare Advantage plans.
