Pioneer ACO Brown & Toland Physicians Goes Live with Humedica MinedShare

Brown & Toland Physicians, one of the nation’s Medicare Pioneer Accountable Care Organizations (ACOs) and a member of the American Medical Group Association’s Anceta collaborative, took another important step in solidifying its coordinated care model with the successful launch of the Humedica MinedShare analytics platform. The analytics platform aggregates and analyzes vast amounts of clinical data from the electronic medical record and incorporates claims data, enabling Brown & Toland’s 1,500 physicians to better coordinate care and drive improvement in patient outcomes.

In the Pioneer ACO program, designed by the Centers for Medicare & Medicaid Services (CMS), organizations are charged with the task of improving care for patients while reducing costs. The model relies on providers being able to closely monitor and manage care by focusing on patient outcomes. A total of 32 healthcare organizations across the country are participating in the Pioneer ACO program.

In addition to the Pioneer ACO, Brown & Toland also is participating in an ACO with Blue Shield for commercial City and County of San Francisco employees and retirees and with Cigna in a collaborative accountable care initiative for PPO patients.
