announced today that it has entered into an agreement with LIFE Pittsburgh, a program of All-Inclusive Care for the Elderly (PACE), which is a specialized Medicare Health Plan.
LIFE Pittsburgh operates a network of three clinics in the greater Pittsburgh area that provide home care, healthcare and adult day care at no cost to qualified older adults who wish to remain independent in their own homes. Through the use of Clear Vision’s RevMaxPro product, LIFE Pittsburgh will be able to better identify and manage the chronic conditions of their PACE membership so they can maintain their independence and enjoy a safe and active life in the community.
RevMaxPro is one of healthcare’s most powerful and easy-to-use data intelligence tools in identifying patients with chronic conditions, pinpointing gaps in treatment, and monitoring the reporting of clinical data to the Centers for Medicare and Medicaid (CMS).
