announced today that it has entered into an agreement with LIFE Pittsburgh, a program of All-Inclusive Care for the Elderly (PACE), which is a specialized Medicare Health Plan.

LIFE Pittsburgh operates a network of three clinics in the greater Pittsburgh area that provide home care, healthcare and adult day care at no cost to qualified older adults who wish to remain independent in their own homes. Through the use of Clear Vision’s RevMaxPro product, LIFE Pittsburgh will be able to better identify and manage the chronic conditions of their PACE membership so they can maintain their independence and enjoy a safe and active life in the community.

RevMaxPro is one of healthcare’s most powerful and easy-to-use data intelligence tools in identifying patients with chronic conditions, pinpointing gaps in treatment, and monitoring the reporting of clinical data to the Centers for Medicare and Medicaid (CMS). Medical groups and Medicare Advantage plans nationwide have come to rely on RevMaxPro to isolate patients with chronic conditions that are missing assessments, create patient evaluation forms to ensure chronic conditions are addressed, and align revenue with expected costs of care. RevMaxPro also provides access to industry benchmarking data that will allow LIFE Pittsburgh to compare its performance to industry norms.

“With RevMaxPro, LIFE Pittsburgh will be better able to isolate seniors with chronic conditions – such as heart disease, stroke, cancer, diabetes and arthritis – and pinpoint gaps in reporting and treatment,” said Tom Peterson, president and CEO of Clear Vision. “Doing so will not only assist LIFE Pittsburgh in conducting the kind of care-management programs that enhance an individual’s quality of life but will also allow LIFE Pittsburgh to quickly and easily make the connection between services delivered and CMS revenue received.”

PACE is a comprehensive-care delivery system that allows those who would be traditionally thought of as nursing home residents to continue living at home while receiving services. It is believed that this model of care will become increasingly important as the concept of medical homes evolves and the emphasis for care continues to shift away from more institutional settings.

Founded in 1999, LIFE Pittsburgh is a community-based alternative to nursing home care and assisted living. Through the LIFE Pittsburgh program, frail older adults can remain in their own home and receive a coordinated plan of care that enables them to maintain their independence and enjoy a safe and active life in the community.
