The most common patient safety error involves medication, and it may be the easiest to avoid with a little preparation. In an emergency, it’s important for healthcare providers to know the drugs and supplements a person takes, the dosages and the reason for taking them, because some drug combinations may cause harmful complications.

Healthcare professionals call this information “medication reconciliation.” The Joint Commission made it a National Patient Safety Goal. The Institute for Healthcare Improvement targeted medication reconciliation as part of its “100,000 Lives Campaign.” And yet, few people are aware that drugs meant to keep them healthy can turn hazardous in unintended duplication of doses or in combination with medications administered in emergencies.

GlobalMed suggests people keep a list of the medications and their dosages and any allergies for each member of the family in one central location in the home. The downloadable is designed for this purpose and is available from the Arizona Osteopathic Medical Association. Once filled in and printed, this two-page form can be attached to the refrigerator with a magnet for quick and easy access. Adults with chronic disease should print out a second copy to carry with them.
