, a leader in ambulatory healthcare IT solutions, today announced that Nature Coast ACO, a Medicare Shared Savings Program , is expanding its relationship with the company to include , coordinating care across the continuum as well as evaluating population health and quality improvement activities.

“Nature Coast continuously looks for tools to help improve the quality of care for its patients,” said Dr. Alex Villacastin of Nature Coast ACO. “As an ACO, we require a community-wide platform to seamlessly connect medical providers along the care continuum. eClinicalWorks had the only technology solution with all of the pieces we need in this initiative. The company has the technology and has shown itself to be a true partner in aiding us in our goals.”

The organization currently uses the eClinicalWorks comprehensive electronic health records (EHR) solution. According to a May 2013 , 95% of organizations with or considering an ACO or PCMH believe that integration with the EHR system is important to create quality outcomes for patients, 82% rating this as “very important.” Additional features noted in the survey as important for creating quality outcomes include physician alerts for high-risk patients within the EHR as well as care-planning and coordination tools.

“Nature Coast ACO remains ahead of the curve in its use of technology to enhance care of its patients,” said Girish Navani, CEO and co-founder of eClinicalWorks. “eClinicalWorks CCMR extends the benefits of most EHRs and offers closed-loop analytics to help organizations look at their data on a community level, while giving physicians access to it at the point of care. We look forward to furthering our partnership with Nature Coast ACO.”

eClinicalWorks CCMR is designed to provide visibility into a patient’s care across all settings, facilitate smooth transitions of care, engage patients and providers in preventative care, improve outcomes among populations and reduce costs. A vendor-neutral, open network securely connects ambulatory EHRs with hospital in-patient systems and payor claim feeds, including Centers for Medicare & Medicaid Services (CMS), to provide cost and utilization for services delivered to patients.

Included in CCMR, eClinicalWorks Community Analytics™ uses closed-loop analytics to deliver reporting, alerting and messaging capabilities necessary to manage Population Health. Dashboards can be shown at the community level to determine and monitor adherence of clinical process and outcomes, for including medication recalls and chronic disease management. Care coordination and analytics work together for a collaborative care framework.
