The Colorado Department of Health Care Policy & Financing and Treo Solutions will be presenting first-year results of the state’s Accountable Care Collaborative (ACC) at Healthcare IT Connect on Tuesday, May 21 (2:55 p.m.) at the Four Seasons in Baltimore. Richard Keller, vice president of government programs for Treo Solutions, and Joel Dalzell, section manager, health data strategy for the Colorado Department of Health Care Policy & Financing (HCPF), will share the ACC program’s FY 2011-2012 findings.

“The Colorado Accountable Care Initiative is a Medicaid reform initiative that is focused on improving health outcomes through better coordinated care while reducing costs. Actionable data and analytics play a pivotal role in achieving these goals,” said Dalzell. “In the first full year of the program, we’ve experienced reductions in unnecessary hospital and ER visits. In fact, we have estimated that the program is also demonstrating cost savings, despite the early program focus on infrastructure and capacity building.”

In 2011, Treo Solutions was selected as the Statewide Data and Analytics Contractor (SDAC) to the Colorado ACC. As the SDAC, Treo creates actionable and accessible data analytics that are delivered through Treo’s proprietary web portal. This information enables Primary Care Medical Providers and Regional Care Collaborative Organizations to monitor and track performance, and gives the state agency the ability to monitor and track the overall success of the program.

“We know that to successfully run the Colorado ACC, transparency, collaboration and action are critical,” said Keller. “To that end, we ensure that the data and dashboards we provide deliver the same information to the various users, support changes at the network, system or member level, and help prioritize and target patients based on need.”

Since 2002, Treo Solutions has been driving transformation in healthcare through alignment, collaboration and transparency leveraging enhanced data to deliver value. Treo’s highly scalable tools, optimized claims database of over 45-million covered lives, and expertise in collaborative care logistics empowers clients to make value-informed decisions to create new risk-sharing and total cost-of-care models. Treo’s focused experience and agility allows clients to anticipate—and rapidly react to—a constantly changing market.
