announced today that it has entered into an agreement with MedCare Quality Medical Centers that will help affiliated physicians better identify patients with chronic conditions and then provide these patients the programs and services needed to enhance their quality of life.

This will be made possible through the use of Clear Vision’s RevenueMax, one of the healthcare industries most powerful and easy-to-use data-intelligence tools. With RevenueMax, physicians can isolate Medicare Advantage patients with chronic conditions – such as heart disease, stroke, cancer, diabetes and arthritis –and pinpoint gaps in reporting and treatment. Such efforts have shown to lead to improved outcomes for the patient while optimizing revenue for the provider.

“Chronic diseases are among the most common, costly and preventable of all health problems in the United States,” Tom Peterson, president and CEO of Clear Vision. “RevenueMax is a turnkey tool that helps ensure that all of a patient’s chronic care needs are being identified and properly addressed based on real-time data that physicians readily have at their fingertips. And from a business perspective, evenueMax allows physicians to quickly and easily make the connection between services delivered and CMS revenue received.”

Seven out of ten deaths in America each year are from chronic conditions, making identifying “gaps in care” a high priority everyone. Doing so involves understanding the needs and medical history of each patient as well as looking for patterns that just don’t make sense. For example, most seniors live with two or more chronic conditions so the medical chart of an 80-year-old showing no such conditions may mean that this patient has not been appropriately evaluated and treated. RevenueMax can assist in this effort.

MedCare Quality Medical Centers is a Miami, Florida-based, managed care organization currently operating two centers with seven physicians on staff. The company plans on opening three additional centers in October, each strategically located in key Florida markets that have a high density of Medicare members.
