, a leading provider of data-driven healthcare transformation solutions, today announced a new partnership with the (CAFP), a not-for-profit organization dedicated to advancing the specialty of family medicine in California and with longtime partner, the (CPCA), the statewide leader representing the interests of California community clinics and health centers and their patients focused on extending proven tools and support to CAFP members undertaking the transformation into a Patient-Centered Medical Home (PCMH). The new partnership aims to transform primary care practices into Patient-Centered Medical Homes using Arcadia and CPCA’s Health Home Accelerator Program and will provide health systems with the most timely, efficient path to PCMH recognition with a target of reducing recognition time by 30 percent.

The standards set by the (NCQA) describe clear and specific criteria and give primary care practices information on how to best organize patient care, work in teams and track patient care over time. The PCMH model facilitates partnerships between patients and their physicians, and when appropriate, the patient’s family. National health care reform legislation calls for implementation of the PCMH model of care, resulting in national and state payers, as well as government agencies, aligning reimbursement and grant requirements with PCMH recognition. With the NCQA PCMH program as the focus for improving primary care, this partnership will provide CAFP members with the tools necessary to expedite the transformation of primary care practices.

By combining tools and transformation expertise from Arcadia and CPCA, CAFP will provide their members with a full suite of tools and methodologies that will enable them to accelerate their path to PCMH. CAFP members will have access to Arcadia’s web-based PCMH Accelerator Portal, which guides providers through the process of transforming into a PCMH, as well as trained coaches with PCMH transformation expertise in many diverse clinical settings.

Arcadia’s Patient Centered Medical Home Accelerator Portal is a web-based platform that combines efficient technology, a library of PCMH documentation guides, direct interface with NCQA, and hands-on clinical coaching to guide family physicians through the complex processes of practice transformation and NCQA recognition. The easy-to-use technology combined with experienced in-office practice coaches help practice teams develop a “Transformation Roadmap” tailored to the practice, outlining the steps necessary to efficiently transform and achieve NCQA recognition.

Using the practice-specific Transformation Roadmap as a guide, the Arcadia Accelerator Portal automatically tracks and reports practice progress in real-time providing visibility and allowing the team to stay on track. Coaches will be available on-site and virtually to support and drive transformation efforts to ensure the changes being implemented are timely, sustainable, and well-aligned with organizational objectives and NCQA standards.
