Virginia Hospital & Healthcare Association (VHHA), an alliance of 37 member organizations representing 110 hospitals throughout Virginia, through its shared services affiliate (VHHA Services), responsible for identifying technology solutions for Virginia’s hospitals and health systems, and ., a pioneer in using mobile applications to deepen the patient’s role in improving outcomes, today announced that they have partnered to provide VHHA member institutions access to Axial Exchange’s interactive health management application. As a part of this partnership, Axial Exchange’s patient engagement application will be available to all VHHA members for individual purchase, allowing these organizations to address many of the healthcare-specific IT requirements around patient engagement and meaningful use.

With Axial Exchange’s applications, Virginia hospitals and health systems will be able to provide patients access to their personal health information in an easy-to-use interface, helping them learn about their medical condition(s), set medication reminders and alerts, and track progress against designated care plans. Among other benefits, the application also provides interactive health trackers to monitor weight, glucose, blood pressure, medication adherence, and other metrics that are essential to health and wellness.

“Our hospitals and health systems are dedicated to continuously improving the health and wellbeing of the communities they serve. Emerging technologies such as mobile health applications to improve patient engagement enhance these efforts,” said David Jenkins, Vice President of VHHA Services. “After a thorough review of solutions, Axial Exchange’s patient engagement application distinguished itself with its intuitive design, interactive content, and wealth of patient education information.”

“This partnership truly exemplifies the growing demand for innovative, accessible mobile health applications to enhance patient-provider communication,” said Joanne Rohde, CEO of Axial Exchange. “We are proud to have been selected by VHHA as a trusted partner, and we look forward to working with VHHA members to improve patient engagement initiatives throughout the Commonwealth.”

Axial Exchange’s solutions are used by more than 70 provider organizations across the U.S. and focus on improving patient engagement and clinical outcomes. Axial Exchange won first prize in the U.S. Department of Health and Human Services’ Partnership for Patients initiative on the Ensuring Safe Transitions from Hospital to Home Challenge run by the Office of the National Coordinator (ONC).
