, formerly the Healthcare business of Thomson Reuters, today announced the availability of a new Electronic Intervention Form Exchange feature for its Micromedex®360 Care Insights Platform, the technical foundation for its products. 360 Care Insights is a that provides clinicians with a consolidated dashboard and daily work lists for patients in the hospital who are at-risk, and those who are good candidates, for early intervention. It includes proactive alerts for critical new values to help clinicians reduce a patient’s risk of infection, adverse drug events, and other potential complications. 360 Care Insights has the flexibility to work with various hospital information systems (HIS). Using pre-implemented patient-profiles built to encapsulate CMS case definitions for criteria, 360 Care Insights monitors data and aggregates pertinent information including, admissions, lab results, observations, medications, transcribed reports, and other patient-specific information, to provide a consolidate 360 degree view.

Available now, products in the 360 Care Insights Suite, all of which leverage this shared technology platform, will export intervention forms completed in the web, mobile or tablet version of the products. The exported forms are available for uploading by a hospital’s information system into the patient’s electronic health record. For example, , using Infection Prevention and Pharmacy Intervention patient-specific forms, are exported to a designated file location and transferred into a patient’s electronic health record. This data exchange capability saves hospital staff, including pharmacists, infection preventionists, care managers, nurses and physicians, time and will help reduce errors by allowing for interventions to be documented just once. It will also provide a complete record of patient encounters for the hospital.

This single, secure dashboard can be accessed online or via mobile devices. The real-time solution can also provide back-up during hospital systems downtime for patient care, allows for bedside intervention documentation and electronic . 360 Care Insights also includes embedded Micromedex® Clinical Knowledge for Clinical Decision Support (CDS), the same Micromedex Clinical Knowledge that can be integrated into everyday tools to support the newly implemented .

Truven Health Micromedex Solutions will exhibit at the upcoming 2012 Annual Association of Hospital Pharmacists (ASHP) Midyear event and will have Clinical Surveillance and Patient Intervention application specialists on-hand at booth #845 to demonstrate the enhanced capabilities.
