Skylight to Unveil CareNavigator™ at HIMSS 2012

First Interactive Patient Care Solution to Bridge Continuum of Care Demonstrated in Booth 1974

SAN DIEGO--(BUSINESS WIRE)-- In order to help hospitals and healthcare organizations enhance the processes needed to succeed in a new era of performance, quality and accountability, Skylight is introducing its fully integrated solution, Skylight CareNavigator™ at HIMSS ’12. From pre-admission to the in-patient stay through post-discharge, CareNavigator connects the patient directly to the hospital and clinicians which provides continuing condition-specific education, reminders of key care events, and a constant health resource across the continuum of care.

Leveraging the feature set of their highly regarded Interactive Patient System for the in-patient environment (formerly called Skylight ACCESS®) and the connectivity and services from their ambulatory solution (formerly called iCarePassport™), Skylight has integrated their product into a new streamlined solution called CareNavigator. With over 70 hospitals achieving an established track record of success with improvements in HCAHPS and other metrics, Skylight is the first company to develop, deploy, and achieve documented results with a complete interactive patient care, information, and services product across the care continuum.

Available today, Skylight CareNavigator delivers seamless interoperability with existing healthcare information systems — including integration of order entry all the way through the tracking of health education viewings in all major EMR systems. It includes patient-centered education, hospital information, and other vital facts to help people prepare for their procedure and hospital stay, while also assuring patients’ informational needs are met after hospitalization.

CareNavigator was built with an understanding that changing reimbursement models will impact how care and education are delivered to a population that is less likely to be readmitted. This occurs as patients are empowered with tools for communication, engagement, and condition-specific education.

“The new reality for healthcare organizations is that care doesn’t begin when a patient is admitted and doesn’t end when a patient is discharged,” said Kevin Meek, RN, BA, MHI, Director of Orthopedics and Neuroscience at Mercy Gilbert Medical in Gilbert, AZ. “Anticipating this shift, Skylight has developed a groundbreaking system that helps patients navigate their journey of care with essential bridges between pre-admission, the hospital stay, and discharge. These bridges make the concept of the continuum of care a reality by managing information and engaging the patient with information and education specific to their episode of care.”

In addition to patient-centric information and support for the clinical workflow, CareNavigator includes a comprehensive reporting system that helps healthcare organizations easily manage their participating covered lives to assist with reporting quality and functional outcomes.

“As surgical practices, hospitals and health systems look for new ways to solidify positions of leadership and maximize performance, we designed CareNavigator to not only improve the patient experience, but also to attach real-world work-flow scenarios and practical programs that have shown dramatic increases in HCAHPS scores and other quality measurements,” said Tom Klopack, CEO of Skylight Healthcare Systems. “We welcome the opportunity to transfer this knowledge gained from our clients who have achieved measurable ROI — including significantly improved HCAHPS scores plus improvements in safety and core measures — to healthcare organizations striving to prepare for the transition to value-based purchasing and the Hospital Readmissions Reduction Program beginning later this year.”
