Saint Francis HealthCare Partners Selects Phytel to Support Its Medical Home Initiative

Innovative Health System Leverages Technology to Automate Care Coordination and Assembles Medical Home Conference to Educate Physicians, Employers, Payors and Healthcare Professionals in Connecticut

DALLAS--(BUSINESS WIRE)-- Saint Francis HealthCare Partners announced today that it has selected Phytel to help achieve NCQA recognition as a patient-centered medical home and will be rolling out Phytel’s unique care coordination and population health management solution, Proactive Patient Outreach, in November. As part of the continual effort for quality improvement, Phytel is sponsoring the 2010 Annual Conference presented by Saint Francis HealthCare Partners and the Connecticut Institute for Primary Care Innovation, taking place on October 26, 2010 at the Hartford Marriott in Farmington, Conn.

The conference will educate healthcare professionals about the patient-centered medical home, the impact of information technology and health information exchange on building the medical home, and the medical home’s role in accountable care organizations (ACOs) and EHR meaningful use. Keynote speakers include leading experts, such as Paul Grundy, M.D., M.P.H., FACPM, president of the Patient Centered Primary Care Collaborative; Pauline Chen, M.D., author and columnist for the New York Times; Gregory Makoul, Ph.D., chief academic officer/senior vice president for Innovation and Quality Integration at Saint Francis Hospital and Medical Center; and Johann Chanin, director of product development for the National Committee for Quality Assurance (NCQA).

“The emerging medical home model will continue to play a crucial role in shaping patient care today and in the future,” said Jess Kupec, president and chief executive officer of Saint Francis HealthCare Partners. “Our conference will bring together healthcare innovators like Phytel and practitioners to cultivate in-depth discussion around this important topic. We succeed through the continual improvement of quality and patient outcomes, and the efficient management of clinical and financial resources. Having Phytel’s participation in this area, we believe, will offer additional guidance during the conference, and as we work to deliver personalized, patient-centered care to our patients and their families – both inside and outside of our facilities.”

Phytel’s Proactive Patient Outreach system creates an electronic registry of patients who require preventive and chronic care, based on nationally validated, evidence-based protocols and data from the organization’s electronic health record and practice management system. The technology identifies gaps in care, triggers automated messaging about recommended visits, tests or procedures, and tracks the results, including quality outcomes data for quality reporting.

“With Phytel’s strong focus on delivering coordinated care and population health management, it made perfect sense for us to partner with Saint Francis HealthCare Partners on its efforts around medical home,” said Steve Schelhammer, chief executive officer of Phytel. “We look forward to working together to elevate the importance of the patient-centered care model, not only through a public forum, but also within the organization. Through education and collaboration from all points in the industry, we will help organizations achieve a new standard of healthcare quality and practice.”
