Phoebe Health Partners Selects Valence Health to Improve Patient Care and Bottom Line through Population Health Management and Clinical Integration Solutions

Together, Valence Health and Phoebe Health Partners, an Albany, GA-based organization, will provide common clinical infrastructure and data information to PHP’s physicians.

PHP participants will have access to a clinically integrated network that includes information from more than 400 community physicians, 12 hospitals, and numerous other healthcare providers. Valence Health will help ensure high quality care and control costs through common quality standards linked across a common . Once integrated, all providers within the network will have access to a patient’s complete history of services received within the network and gaps in care will be clearly indicated as opportunities for improved compliance.

Under the agreement, Valence Health will also provide PHP the solution that combines data from over 50 practice management and hospital information systems and dozens of EMRs with lab, prescription and other data sources, to provide a comprehensive clinical view of PHP member patients. Once operational, the system will allow care providers and coordinators to identify customized patient populations based on combinations of clinical information – such as conditions, gaps in care per evidence-based guidelines, lab results, vitals and more.
