a wholly owned subsidiary of . (NASDAQ: QSII) and a leading provider of healthcare information systems and connectivity solutions, announced today it has entered into an agreement with Southwest Community Health Center, Inc. for deployment of , and across its 12 locations throughout Southwest Connecticut.

Southwest Community Health Center (SWCHC), Inc. is a Federally Qualified Health Center (FQHC) with more than 20,000 patients averaging more than 115,000 visits per year. Its 220 employees address various social determinants of health by providing comprehensive, culturally competent healthcare and social services. Primary and preventive care services such as medical, dental, and behavioral health are provided to individuals of all ages, regardless of their ability to pay.

As a Federally Qualified community health center with multiple grant awards, SWCHC requires more than standard clinical and financial systems to meet its robust reporting requirements. Utilizing NextGen® solutions, SWCHC will have the integrated reporting utilities to support the variety of documentation needed to satisfy federal and state regulations and funding requirements, as well as disease collaboratives, auditors, and private payers.

The Center will leverage NextGen Practice Management to increase revenue through enhanced charge capture capabilities, elimination of document storage fees and improved operational efficiencies. NextGen Ambulatory EHR and NextGen EDR will integrate SWCHC’s clinical, scheduling, pharmacy and dental services to create a comprehensive medical management platform. This in turn, supports the centers’ efforts and its plans to meet Level 3 National Committee for Quality Assurance (NCQA) (PCMH) criteria.

SWCHC is near completion of its new 22,000-square-foot multiple-use health center and supportive housing residence in Bridgeport, CT. This building project is in conjunction with the Bridgeport Housing Authority and will create 30-40 units of new affordable housing and a health center that will allow SWCHC to house medical, dental, behavioral health and administrative functions in one state-of-the-art facility.

“The latest changes to the state Medicaid program require us to transform our healthcare delivery by adopting quality care models such as PCMH,” said Katherine Yacavone, president and chief executive officer of SWCHC. “We’re adapting to these new models by incorporating NextGen Healthcare’s product suite into our platform to provide seamless integration of all medical components of care onto a single system developed specifically to meet our community health center needs. Our partnership with NextGen provides us with all the tools we need to meet Meaningful Use requirements and achieve NCQA accreditation.”

“In this era of and constant change, those who understand where they are, how they are performing and where they need to improve are better equipped to qualify for programs such as Pay for Performance, PCMH or an ACO,” said Scott Decker, president of NextGen Healthcare. “Leveraging our customizable technology and services will help SWCHC address their distinct clinical and business challenges and aid in advancing their objective to deliver quality care and provide a sought-after medical home for the patient populations they serve. This is an exciting time in healthcare and we tailor our software to help clients meet specific goals, objectives, and care plans.”
