Health Dialog Announces Collaboration with Australian Department of Veterans’ Affairs

Chronic Condition Support to Begin in June 2011

BOSTON--(BUSINESS WIRE)-- Health Dialog today announced that its sister company, Bupa Health Dialog Australia, has contracted with the Australian Department of Veterans’ Affairs (DVA). Bupa Health Dialog Australia, with the support of Health Dialog in the US, will provide analytic services to identify veterans and dependents with chronic conditions who are most at risk of hospitalization. These services will be delivered as part of the DVA’s four-year $152.7 million Coordinated Veterans’ Care Program.

General practitioners (GPs) will work closely with identified individuals to help them take a more proactive role in their care. The goal is to have an estimated 17,000 veterans facing one or more of five chronic conditions – congestive heart failure, coronary artery disease, pneumonia, chronic obstructive pulmonary disease, and diabetes – participate in the program by 2014.
