Cognify, Inc. Selects Greenway's PrimeSUITE to Advance Integrated Care Coordination

Combined Technologies Streamline Elder Care Wellness, Satisfying CMS Payer Requirements and Providing Innovative, Comprehensive Solutions to Caregivers

CARROLLTON, Ga., April 18, 2011 /PRNewswire/ -- Greenway Medical Technologies, Inc. today announced the integration of its electronic health record (EHR), practice management and interoperability solution PrimeSUITE® 2011 with Cognify, Inc.'s web-based Participant Tracking System that monitors care plan continuums.

Together, the technologies provide an advanced and robust solution to customers such as Denver, Colo.-based Total Longterm Care Solutions (TLC), one of the nation's largest elder care providers within the Centers for Medicare and Medicaid Services (CMS) Programs of All-Inclusive Care for the Elderly (PACE®) program.

PACE regulatory requirements encompass clinical care planning, Medicare and Medicaid covered services, and those deemed necessary by interdisciplinary medical teams such as homecare, transportation, scheduling and prescriptions.

Cognify's Participant Tracking System combined with PrimeSUITE's functionality allows more than 700 current TLC caregivers the ability to easily coordinate clinical and administrative care plan data to increase efficiencies, lower costs and adhere to PACE program compliance.

"Electronic care planning in tandem with clinical staff and federal regulations allows us to clearly document and integrate the many aspects of complex care," said TLC Vice President for Quality Susan Grayson, RN, BSN, MA. "This system has been working reliably and the customer service to make adjustments as new ideas develop has been tremendous."

Based on CMS-sponsored models of care and widely considered the 'original Accountable Care Organizations', PACE serves individuals who are age 55 or older, certified by their state to need nursing home care but are able to live safely in the community at the time of enrollment, and live in a PACE service area. Although all PACE participants must be nursing home-eligible to enroll in PACE, the programs are so effective that only about seven percent of PACE participants nationally reside in nursing homes.

"We are excited to partner with Greenway Medical Technologies to provide the most advanced care possible to participants in the program," said Jim Hogan, vice president of Managed Services for Cognify, Inc. "Greenway's PrimeSUITE is a perfect match for our Participant Tracking System, and together they create a solution that is simple from a user-interface perspective, but remarkably powerful. Such a broad spectrum of care requires specialized software to host both medical records as well as a wide variety of administrative information."

"Greenway is dedicated to creating a community approach to healthcare in America, and Cognify and its PACE clients have similar objectives," said Tee Green, president and CEO of Greenway®. "Participants thrive in the PACE environment, and Greenway is honored to be part of a program that improves the quality of life for our nation's senior citizens. We look forward to expanding our partnership with Cognify in order to reach an even larger segment of this population."
