In an effort to cut down on hospital readmissions, the Centers for Medicare & Medicaid Services (CMS) last week awarded grants to seven sites in the new Community-Based Care Transition Program, under the Partnership for Patients program. With aims to reduce readmissions by 20 percent over three years, reports The Boston Globe, the pilots include regions across the country. Among the participants are Atlanta's Piedmont Hospital, Merrimack Valley of Massachusetts, and the Healthcare Improvement Collaborative. CMS announcement (.pdf)
