BlueCross BlueShield of Tennessee Selects Care Team Connect’s Population Management Platform to Power Its Statewide Patient-Centered Medical Home Program

Care Team Connect (CTC), the leading provider of care management platforms to power innovative population health strategies, announced today that its population health management platform was selected by BlueCross® BlueShield® of Tennessee. Care Team Connect’s secure, web-based platform will be used by provider-based care coordinators across the state to help improve overall quality of care while also reducing costs.
In addition to enhancing patient care for Medical Homes, the platform will also enable BlueCross to streamline its Pay for Performance (P4P) programs. Care Team Connect will enable BlueCross to measure, monitor, and manage performance against a core set of measures.
